401k Termination Form & Process

After my contract has ended, what do I do with my 401k?

Below is a step by step guide on how to terminate your 401k with Epitec


Step 1- Complete the appropriate form (attached below), if you are enrolled in a ROTH plan please complete the ROTH Form. Signature on the form must be in ink. An electronic signature will not be accepted. 

Step 2- Send back the competed form to Epitec for the plan administrator’s signature.

Step 3- Once the form has been signed by both parties Epitec will submit to ADP and send you a confirmation number.

After the form is processed by ADP a check will be sent to the address that you populate on the Termination Form.

  • Lump sum – 20% withholding and any state tax if applicable, 10% penalty applies if under 59.5 (ADP does not withhold the penalty)
  • Rollover - 0% withholding, no penalties. There is a $55.00 distribution fee when rolling money out of the plan
  • Combination of the 2 (lump sum and rollover) – for the portion that is a lump sum, 20% withholding, plus any state tax if applicable and 10% penalty if under 59.5
  • Partial – you do not take out all of the money, some money remains in the plan.
