If you need to rollover your 401k INTO Epitec's plan:
Please fill out this form attached.
You will need to sign the form in ink and scan it back to us. We can then have our plan administrator sign the document. Please submit a request through the Employee Portal with the signed form.
The plan number is 471510, if you have any further questions Customer Service line for Epitec at 866-695-7526. Please reference the plan number when you call.
Once both signatures are obtained, we will send this to ADP for processing. Both the check and form must be included when this is sent to ADP, so please make sure to include the check (addressed) in the request.
