In regard to Short Term Disability, if you are actively enrolled, you can complete a Short-Term Disability Claim document on the Unum website around this time as well. It will be required for you, your doctor, and Epitec to fill out sections of this form. Once it is completed, Unum will review this l to determine your claim.
For further information on Short Term Disability, please refer to the National Benefits Guide or Michigan Benefits Guide. Unum may determine a pre-existing condition with your pregnancy upon investigation of your claim. (If the pregnancy was known during enrollment of Short-Term Disability Benefit).
