i am approaching my due date and I want to discuss maternity (Short term disability) options that are available to me and also from when I can start using them.
Step-by-step guide
In regards to leave, Epitec does not have a Maternity Leave Policy. However, once you hit your 1 year tenure of employment, you will become eligible for FMLA Leave. FMLA legally holds your position open while you are on leave for up to 12 weeks. You will be required to fill out certain FMLA documents about a month prior to your tentative leave date. Please reach out to us at this time so we can send you the proper documentation.
In regards to Short Term Disability, since if you are actively enrolled, we will send you a Short Term Disability Claim document to fill out around this time as well. It will be required for you, your doctor, and Epitec to fill out sections of this form. Once it is completed, Epitec will send it to Prudential to determine your claim.
For further information on Short Term Disability, please find attached the benefit guide and go to page 15 .Prudential may determine a pre-existing condition with your pregnancy upon investigation of your claim. (If the pregnancy was known during enrollment of Short Term Disability Benefit). I have also attached the STD claim form if needed
