Explaining Direct Deposit Forms

This is how to fill out the Direct Deposit Authorization Form. 

Instructions

Where it says “specified amount” you should have a designated dollar amount in ONE of the bank accounts and not the other. This way, if the check amount ever varies for some reason you will get the exact amount earned rather than the amount you put on the form.

For example:  say you would like to add $100 of every paycheck to a second bank account. You would put that $100 on the specified amount for that bank account you want it to go in.  Please refer to the example image attached for reference.


If you only have one bank account. Please fill out Direct Deposit Account #1 and leave #2 blank.
