These plans are VOLUNTARY and are additional to the Basic Life & Basic AD&D that is automatically enrolled.

If you would like to increase the coverage outside of the Basic Life & Basic AD&D amount (15,000) you need to select an "additional amount" under voluntary employee life or voluntary AD&D. These plans are VOLUNTARY and are employee paid.

You cannot just have 0 for additional amount as it does not qualify for this enrollment.

If you do not select an additional amount, this benefit will be terminated as it is for a voluntary additional amount over the basic life and AD&DThese are in addition to the Basic Life & Basic AD&D that is provided to each eligible employee.


Employee Voluntary Life
Benefit Amount: increments of $10,000
Non-Medical Maximum Benefit:
$200,000
Maximum Benefit: the lesser of 5x Annual Base Earnings or $200,000


Spousal Voluntary Life
Benefit Amount: increments of $5,000
Non-Medical Maximum Benefit: $10,000
Maximum Benefit: $100,000
Spouse amount cannot exceed 50% of the employee’s Supplemental Life benefit.


Dependent Child Voluntary Life
Benefit Amount: Increments of $2,000
Non-Medical Maximum Benefit: $10,000
Maximum Benefit: $10,000
Child amount cannot exceed 10% of the employee's Supplemental Life Benefit.
